If I play a Doom–Attack and everyone has a Guardian (or Lighthouse, Champion, or Moat), do we reveal and discard the next Hex anyway and no one is affected by it? Or do we leave the Hex pile unchanged?

You discard it.

If everyone has a Champion out, might as well put the Hexes back into the box.

(unless of course Cursed Village or Leprechaun are also in the game)

No no. You must see them. Experience the thrill of Champion protecting you from each one in turn.

It helps to say "look at what you could have won" while doing so.

...though that may only make sense to Brits who remember tacky eighties game shows.

That show (and catchphrase) was actually exported to other countries, including mine  :)

Or get the sadist joy out of not playing your Skulk, specifically to deny your opponents of said thrill.
